Health Assessment

 

New for 2026!   To earn a Live Life Well Incentive in 2027, you must complete the MediKeeper Health Assessment along with a State-sponsored health screening, self-reporting your Nicotine Free status, and self-reporting completion of an Eligible Provider Visit by October 31, 2026.

  • The MediKeeper Health Assessment is a personalized, systematic questionnaire designed to help you identify risk factors, provide individualized feedback, and link you with interventions to promote health, sustain function and/or prevent disease.
  • The Health Assessment is entirely confidential and takes 15 minutes to complete.

Complete the Health Assessment

Start your Health Assessment today by logging into the MediKeeper Wellness Portal.

  1. Visit LiveLifeWellMT.medikeeper.com.
    • If you haven’t already, you must create an account on our wellness portal, Medikeeper. Enter your name exactly as it appears on your BlueCross & BlueShield of Montana medical benefits ID card. Covered spouses or domestic partners must make their own account using their personal information.
    • Once you’ve created your account, you may also use the MediKeeper app, Alaveda, to complete the Health Assessment & other Live Life Well Incentive Activities.
  2. Once you’ve logged in to MediKeeper or the Alaveda app, click on “Health Assessment.”
    • The Health Assessment will take about 15 minutes to complete, and you don’t have to do it all at once. 
    • Previous year Health Assessment results can be accessed by clicking on the “HRA Yearly Comparison” button (not the “Health Assessment” button).
  3. For purposes of earning the Live Life Well Incentive: once you have completed the Health Assessment, click on “Incentive and Rewards,” it will show as “Complete” in the “2026 Live Life Well Incentive” section.

The State Plan offers the LLW incentive program to all enrolled employees (for the purpose of this disclaimer employees includes active employees, retirees, legislators, and primary COBRA participants) and their enrolled spouse/domestic partner.

If you choose to participate in the LLW incentive program, you will be asked to complete a voluntary health risk assessment that asks questions about your health-related activities and whether you have or had certain medical conditions (e.g. cancer, diabetes, or heart disease). You will also be asked to obtain a voluntary blood test for certain conditions (e.g. cholesterol). If you choose to participate, you will receive a reduction in your bi-weekly employee contribution. If you are unable to participate in any of the health-related activities of the LLW incentive program, you may qualify for a reasonable accommodation to earn the incentive. The information collected from your health-related activities will be used to help you understand your current health and potential risks and to offer you services.
