Earn a $200 Healthy Weight Incentive!
The Healthy Weight Incentive is available annually to members of the State Plan and their covered spouse/domestic partner who complete an eligible program and meet the Healthy Weight Incentive requirements.Pre-registration is not required.
Each program is different. Please review the information about each program and decide which, if any, will work for you. More examples of kinds of documentation that qualify is included on the application forms (under the "Healthy Weight Incentive Forms" section).

The Montana DPP is a one-year program that supports healthy lifestyle changes for adults who are at risk for developing type 2 diabetes. Trained lifestyle coaches deliver the program through group meetings at 19 organizations across the state or virtually. Participants gain confidence and learn new life skills.
Diabetes Prevention Program Overview
Adults are eligible to participate in the DPP if they are overweight, have medical clearance, and have one or more of the following risk factors:
You will attend 1-hour group classes for a 16-week series of nutrition and physical activity classes and then 6 monthly classes. You will learn how to make better food choices, be more active, and manage stress or other challenges to help you lose weight.

Q: Why a Healthy Weight Incentive?
A: Weight management is the one of the most important issues that we, the Plan members, want to address. The Healthy Weight Incentive requirements were set based on scientific evidence that shows the greatest rewards for our members and our Plan comes with support from an evidence-based program, regular physical activity, and weight loss of at least 10% (or achieving a normal BMI). The Healthy Weight Incentive is designed to reward members who have put forth the effort needed to achieve the requirements.
Please use your discretion when considering the information provided in these programs and talk to your doctor as needed prior to making any changes.
Q: Is there a specific date I have to start to earn the Healthy Weight Incentive?
A: No, you can begin anytime that works for you. Once you have completed all three requirements, then you can submit your incentive application. However, you may only submit one incentive application per plan year (January 1 - December 31).
Q: I am using another program or working with my doctor to lose weight. Am I eligible to earn the incentive?
A: You can still earn the incentive but you do need to complete all three requirements to earn the incentive, including 4 months participating in one of the three eligible programs. The Healthy For Life Program works well in conjunction with another program.
Q: I already earned the Healthy Weight Incentive. Can I earn it again?
A: Yes, you may earn the Healthy Weight Incentive once per plan year (January 1-December 31) as long as you have met the incentive requirements.
You will need to re-do all three requirements including participating in an eligible program for 4 months, losing 10% of your starting weight (whatever that might be - it is not connected to any previous application you may have submitted) or showing a "normal" BMI, and participating in physical activity of your choice.
Q: I started losing weight before I started an eligible program. Can I still earn the incentive?
A: Yes, as long as you complete all the requirements, you do not have to complete all three at the same time. Just complete all three requirements roughly within the year previous to the date you submit your application.
Please note: we understand that it sometimes takes more than a year to meet all the requirements, especially the weight requirement. It is okay to take more than a year to get there. Just submit your application once you have met all three requirements.
Q: I am at a "normal" BMI or don't need to lose 10% of my body weight to get to a normal BMI. Am I eligible to earn the Incentive?
A: Yes, as long as you meet the other program requirements, a normal BMI will satisfy the weight requirement.
The State Plan offers the incentive program to all enrolled employees (for the purpose of this disclaimer employees includes active employees, retirees, legislators, and primary COBRA participants) and their enrolled spouse/domestic partner. If you choose to participate in the incentive program, you will be asked to complete a voluntary program and provide documentation of meeting the incentive requirements. You will also be asked to obtain voluntary measurements from your health care provider (e.g. BMI). If you choose to participate, you will receive an off-cycle paycheck to the primary plan member, which is subject to taxes. If you are unable to participate in any of the health-related activities of the incentive program, you may qualify for a reasonable accommodation to earn the incentive.
