Free At-Home COVID-19 Tests for State Plan Members
Effective April 4, 2022, the State Plan will no longer provide Medicare retirees coverage for over-the-counter COVID-19 tests, as the coverage is now available via Medicare Part B.
Free federally Issued At-Home covid-19 Tests
Available for all U.S. homes, State Plan membership not required.
- Every home in the U.S. is eligible to order free at-home tests.
- Order tests at COVIDtests.gov.
Direct-to-consumer at-home covid-19 tests
Available to all State Plan members, except Medicare Retirees.
Available from Costco pharmacy (membership not required)
Each order contains 2 boxes with 5 COVID-19 tests per box, for a total of 10 at-home COVID-19 tests.
Place an order, follow these steps:
- Go to costco.com/home-delivery
- Select Mail Order
- Register for a new account or sign into your existing account
- You do not need to be a Costco member to use their mail order service
- Select: New Prescription
- Select: “My doctor will call or fax Costco. I do not have a written prescription”
- In the Physician Information fields:
- Enter Physicians Name: Costco Test
- Enter Physicians Phone: (800) 111-1111
- Enter Physician City: Seattle
- Enter Physician State: Washington
- Enter Drug Information: COVID Tests
- Select shipping option
- Click Next and Confirm Prescription Request
- Click Complete Prescription Request to complete order
purchase At-Home covid-19 Tests from retailer
Available to all State Plan members, except Medicare Retirees.
- Purchase at a Navitus In-Network Pharmacy
- Members will need to present their Navitus Pharmacy ID card at time of purchase at the pharmacy window.
- At-home test cost is processed at point of sale with no out of pocket cost to the member.
- To find an in-network pharmacy, login to your account at navitus.com and click "Pharmacy Search."
- For Purchases Made Out of Pocket
- Members pay for the at-home test up front then submit a claim to Navitus for reimbursement of up to $12 per test.
- Submit reimbursement claims using the OTC COVID-19 At Home Test Claim Form.
- Due to high claims volume, reimbursement claims for at-home COVID-19 tests may take up to 90 days to process.
Reimbursement Details
- At-Home COVID-19 tests will be covered at a reimbursement rate of up to $12 per test.
- Limit of eight at-home tests per State Plan Member every 30 days.
- The quantity limit does not apply to test ordered by a health care provider.
- Tests packaged in multi-packs will be reimbursed accordingly.
- At-home COVID-19 tests must be authorized by the U.S Food and Drug Administration (FDA) to be eligible for reimbursement.
- Not eligible for at-home tests purchased before January 15, 2022.
Medicare retirees
