Healthy for Life Prenatal Program

 

Welcome to the Healthy for Life Prenatal program!  This free and confidential program is available to all State of Montana health care plan members during their pregnancy, and is designed to help you achieve a healthier pregnancy.  Members are encouraged to enroll as soon as they know they are pregnant.

  

Program Benefits:

·         Free generic prenatal vitamins

·         Pregnancy risk screenings

·         Nutrition consultations from a Registered Dietitian

·         Healthy pregnancy booklet

·         On-line maternity resources

·         Post-partum and follow-up screenings

·         Enhanced Benefit: For plan members who enroll in their first trimester and participate in required prenatal screenings.

  • 0% copay (no deductible) for routine, pregnancy-related physician charges including routine office visits, physician delivery charges, and lab charges.
  • 0% copay on first ultrasound.
Program Requirements:

In order to continue receiving the benefits of the Healthy for Life Prenatal program, you will need to keep in touch with our office at least once per trimester.

To Participate:

To participate in the Healthy for Life Prenatal Program, please review the program guidelines, and complete and return the prenatal questionnaire.
