Report Your Activities
- Log into www.myactivehealth.com/som
- New users, make an account using your personal information exactly as it appears on your Allegiance ID card.
- Click the “My Rewards” tab to check and report your 2017 Live Life Well Incentive activities.
- Self-report your Next Steps and Nicotine Free alternative activities (if applicable) by clicking “Work On It,” then click the box next to the activity you completed, and then "Save" at the bottom of the section. Please note: health coaching presentations, suicide prevention classes, and provider visits are all grouped into the category labeled "at least two sessions with a provider, health coach and/or eligible presentations."
***Remember, you must also complete a State-sponsored health screening between November 1, 2016 and October 31, 2017 to be eligible for ANY Incentive.***
Complete and report one of the activities below between November 1, 2016 and October 31, 2017 to qualify for the Next Steps Incentive.
These programs are approved as Next Steps activities because they use proven strategies to address lifestyle factors that will help you manage or lower any of the four At-Risk Levels.
Some of these programs have pre-requisites you must meet to be eligible.
Please consult with your provider before engaging in any new activity or program.

Health screening results for BMI, blood pressure, cholesterol, and blood glucose are predictors of your risk for developing conditions like heart disease, cancer, and diabetes. We want to encourage members to take steps to prevent or control these conditions in order to increase your health and control costs for yourself and the State Plan.
If your results are above the levels indicated above, they will show up as elevated (higher risk level) on your State-sponsored health screening results. Members whose results are above these levels are at increased risk for adverse health conditions.

The State Plan offers the incentive program to all plan members and their enrolled spouse/domestic partner. If you think you may be unable to meet a standard of the incentive program, you may qualify for an alternative program or different means to earn the incentive. You must contact the Health Care & Benefits Division (HCBD) as soon as possible at 800-287-8266 or email firstname.lastname@example.org. We will work with you (and if you wish, your doctor) to design a program with the same incentive that is right for you.
