Nurse Health Coaching

If you have a health condition, you can take advantage of one-to-one coaching from a registered nurse through the Nurse Health Coaching program. It’s confidential, it can help you manage your condition, and it’s available as part of your health benefits – so it’s no cost to you!

You are eligible for Nurse Health Coaching if you have asthma*, chronic kidney disease, COPD, chronic pain, congestive heart failure, coronary artery disease, diabetes*, high blood pressure or high cholesterol.

When you enroll in the program, you will speak regularly with your personal nurse health coach who can help you understand and manage your condition. Your nurse can send you additional educational information and support you when your health is difficult to manage.

Montana Health Centers

For members able to visit a Montana Health Center and who would like to connect in-person.  CareHere's disease management focuses on preventive health care and includes health coaching and primary care visits related to your condition.
  • Cancer
  • Peripheral artery disease
  • High blood pressure
  • Cardiovascular disease (including heart disease, arteriosclerosis, atherosclerosis, and stroke)
  • Diabetes (co-payment reductions with participation)
  • Chronic respiratory diseases (asthma, COPD, emphysema)
  • Rheumatoid arthritis*, osteoarthritis*, gout
  • Joint and back pain

The CareHere Wellness team includes healthcare professionals such as physicians, mental health care providers, physician assistants, nurse practitioners, nurses, dietitians, fitness experts and other wellness personnel to give you the best overall care.


*Other Programs - available in partnership with the Department of Public Health and Human Services

  • Blood pressure management program - free blood pressure cuff and more.
  • Asthma program - co-payment reductions available for certain medications.  Call (406) 243-4647 for more information.
  • Walk With Ease - Introductory walking program for those with arthritis, or anyone who is just getting started.
